Consent is specific and separate
We record separate consent for phone contact, email contact, text messages, marketing communications and the processing of health-related information. Agreeing to one does not agree to the others, and no box is ever pre-checked for you.
What we record
For each consent we store what you agreed to, the date and time, and the version of the notice that was displayed to you.
Text messaging
SMS consent is entirely optional and is never a condition of purchasing insurance. Message frequency varies, message and data rates may apply, and you may reply STOP at any time to stop messages.
Health-related information
Information about doctors, medications and health needs is only requested through a secure intake process, only after we explain why it is needed, and only with your explicit consent. It is stored separately from general inquiry records and is accessible only to authorized personnel.
We never request Social Security numbers, payment card data or complete medical records through this website.
What submitting a form does not do
Submitting a form does not enroll you in any insurance plan, does not guarantee eligibility or approval, and is not medical advice.
Withdrawing consent
Call (313) 850-6033 or use the contact form and tell us which consent you would like to withdraw. We will confirm the change in writing. Withdrawal does not affect processing that already occurred.
